Nominate a person, group or business who has contributed to an inclusive workplace and/or community or championed opportunities for people with a disability.
Nominations will open soon for the Elissa Flanagan ‘Aim High’ Scholarship and Judy Antonio Memorial Award.
There are two ways you can nominate:
1. Download and complete a hard copy Nomination Form (Word Document)
2. Complete the online nomination form below
We recommend you nominate early to allow your nominee sufficient time to submit an entry.

The Awards are an excellent opportunity for you to share and showcase your contribution, commitment and leadership to creating inclusive work communities.
There are seven (7) award categories you can choose from.
Entries can be in the form of a written entry, video or voice recording (as long as your entry answers the three questions related to your chosen award category).
Winners receive a framed certificate and trophy. The winner of the Elissa Flanagan 'Aim High' Scholarship will receive a $2000 voucher towards professional development, training and/or education in their chosen field or pursuit.

3. Submit your written or recorded entry by email by the due date (TBC)
If you cannot submit your Entry by email, please contact the Event Coordinator to discuss an alternative option.
TIP: When you open the Entry Form, download and save it to your computer first before entering any information. This will help you avoid losing any information you enter.
